For the last eight years I have been employed by HB District Health Board as therapist and facilitator at the Springhill Residential Addiction Treatment Centre. Before becoming a therapist I had been a pharmacist and CEO, living in New Zealand since 1984. In 1985 I became the Managing Director of Weleda NZ spending the next ten years building up that company to a multimillion dollar organisation. I served as director and chair on several boards and am currently a director of Prometheus Ethical Finance. Having experienced corporate life at executive level and now as frontline worker in the DHB's Addiction Services, I believe myself to be uniquely qualified to make the concerns of patients and frontline service staff heard at the DHB board table − Please help me to make your voice heard!

I, Otto Ernst Friedrich Mengedoht, being nominated for the position of Board Member of the Hawke's Bay District Health Board, hereby declare that I believe I have a conflict, or a potential conflict, of interest due to any or all of the following matters or reasons:−
I am employed by the HBDHB as a therapist at the Springhill Residential Addiction Treatment Centre;
I am a director of Prometheus Finance Ltd.
To the best of my knowledge and belief, I have no other conflicts of interest with the DHB at the date of my notice of consent to being nominated as a candidate for membership of the board of the DHB.
I do not believe that any conflicts of interest are likely to arise in future, other than the continuation of the conflicts of interest stated above.
